What is the Account Information Change form needed for?
The Account Information Change request form is needed for the following situations:
- Change of the name on your account.
- Change of the business or company name on your account.
- Correction of the date of birth on your account.
- Change/correction to the tax information on your account.
- For minor corrections (1-2 typos) please send a request just with your W9 to Customer Experience. Anything more will require this form as well.
- Change of email address.
- This can be done on your My Account settings menu.
- Adding a co-applicant to your account.
In addition to filling out the associated boxes on this form, you may need to provide supporting documentation. Please see the following information on supporting documentation based on the request being made:

For change of name requests:
- W9/W8BEN (Brand Partners only)
- Driver's License, Marriage License, or Court Petition for Legal Name Change

Business/Company name change/Correction:
- W9/W8BEN
- IRS/CRA Confirmation letter for EIN
- Articles of Incorporation (For a corporation) or Articles of Operation (For an LLC)

Date of Birth Correction
- Driver's License, Passport, Birth Certificate, or Citizenship Certificate

Tax ID Correction
- For SSN correction - W9 or Social Security Card
- For EIN - Articles of Incorporation or IRS/CRA Confirmation Letter
